NYS Institute for Health Transition Training Launches New Web Site


The www.HealthyTransitionsNY.org  Web site is an interactive curriculum and toolkit for youth with developmental disabilities ages 14-25 years, family caregivers, and health care providers. The goal is to raise awareness about developmental disabilities, to improve communication, and to build effective partnerships during the transition from pediatric care to adult health care. The site is ADA accessible and provides automated Spanish language translation. It features video vignettes that demonstrate health transition skills and interactive tools that support collaboration.

The Healthy Transitions curriculum focuses on 10 skills:

1. Scheduling an appointment
2. Getting health insurance
3. Deciding About Guardianship
4. Speaking up at doctor's office
5. Understanding my disability
6. Managing medications
7. Keeping a health summary
8. Looking into service coordination
9. Setting health goals
10. Finding Community Resources

The Healthy Transitions project was developed in 2006 by the New York State Institute for Health Transition Training at the Center for Development, Behavior, and Genetics at SUNY Upstate Medical University with funding from the New York State Developmental Disabilities Planning Council.  The Institute represents a collaboration among pediatricians and family practitioners at SUNY Upstate Medical University and primary care providers throughout New York State.  The New York State Department of Health, Parent to Parent of NYS, Self-Advocates of NYS, the Central New York Developmental Disabilities Services Office, Syracuse University School of Information Studies, and the Burton Blatt Institute at Syracuse University are also founding partners.
 
The mission of the Institute is to promote self-determination, to optimize health and well-being, and to improve access to health care and the quality of health care that is delivered to young people with developmental disabilities aged 14-25 years in New York State.  
 
Training sessions are available throughout New York State to introduce users to the curriculum and tools. The Institute provides 1-hour “academic detailing” sessions at primary care medical offices as well as  ½ day workshops for groups of parents, service coordinators, youths, and interested others.  Continuing education credits (CME CNE etc) will be provided to health care practitioners who participate in workshops or complete the course on-line.
