Future Care Planning Services is a partnership of Lifespan of Greater Rochester, Al Sigl Community of Agencies and ARC of Monroe County. It was established in 2001 through grants provided by the Golisano Foundation.

Future Care Planning Services provides comprehensive life care planning that promotes peace of mind for caregivers and their loved ones with disabilities by offering choices in key areas including financial, medical, legal, residential and other important services.

The Golisano Foundation helped establish the Future Care Community Pooled Trust, a local community pooled trust for people with disabilities and their families. The Future Care Community Pooled Trust serves the Upstate New York area (Monroe and Surrounding Counties).

In 2007, The Foundation funded legal services related to guardianship issues.

The Golisano Life Enhancement Fund was established in 2008 through a gift from Tom Golisano. The Life Enhancement Fund was created within the Future Care Planning Services Community Pooled Trust to provide an opportunity to enrich the lives of upstate New Yorkers with disabilities and special needs. Life Enhancement Funds may be used to help offset the cost of a wide range of goods and/or services intended to enhance the quality of life for a person with a disability. The identified needs must be for the sole benefit of the beneficiary. These may include goods or services related to the beneficiary’s health, welfare or living environment.
